As defined by IDEA 2004 (34 CFR 300.43), transition services refer to a coordinated set of activities designed to support students with disabilities in achieving academic and functional success, facilitating their movement from school to post-school activities. 

With the student’s vision at its core, transition planning is a personalized journey that develops over time. It can include a wide range of action steps, from everyday life experiences supported by family and friends to specialized instruction provided by trained educators.
